*Top line Name + DOB, Bottom line Date + Sign, Bottom RHS GDPR Preferences
JUNIOR MEMBERSHIP
This is any member under 14 years old. One data sheet can be used by up to 2 juniors but we need a separate consent sheet per junior
Page 2 – DATA SHEET required information* :-
*Name, Gender, Relationship, Date of Birth, Address, Email, Sign & initial on RHS ideally where possible by both parents
Page 4 – CONSENTS SHEET required information* :-
*Top line Name + DOB of said Junior, Bottom RHS GDPR Preferences, Bottom line Date + Sign & initial ideally by both parents
TEENAGE MEMBERSHIP
Any member over 14 years old but under 18 years old. One data sheet can be used by up to 2 teenagers but we need a separate consent sheet per teenager
Page 1 or 2 – DATA SHEET required information* :-
*Name, Gender, Relationship, Date of Birth, Address, Email, Sign & initial on RHS ideally where possible by both parents
Page 4 – CONSENTS SHEET required information* :-

FAMILY MEMBERSHIP
E.g. Husband / Wife / Partner / Spouse. Here we need one data sheet + 1 consents sheet
